Despite significant progress in Pakistan’s fight against polio, with cases dropping from 74 in 2024 to just six this year, a troubling rise in vaccine refusals is raising alarm, especially in Karachi. As the federal government prepares for another anti-polio campaign set to begin on April 21, new data reveals that 44,000 people across the country are refusing the vaccine, with a staggering 34,000 of these refusals coming from Karachi alone.
The situation is most concerning in District East of Karachi, where nearly 80% of the refusals are concentrated. Federal Health Minister Mustafa Kamal noted that of these refusals, 15,000 come from Urdu-speaking households and 10,000 from Pashto-speaking families. This demographic breakdown not only points to specific areas of concern but also reveals a deeper crisis rooted in mistrust and misinformation surrounding the polio vaccine.
Conspiracy theories, such as claims that the vaccine causes infertility or is part of a foreign plot, continue to gain traction in these communities. These false narratives are fueling the refusal to vaccinate children and jeopardizing Pakistan’s near-eradication of polio. As the country prepares for its upcoming vaccination drive, it is clear that the campaign must go beyond merely administering vaccines. It must become a broader initiative focused on rebuilding trust and dispelling myths.
To overcome this hesitancy, polio teams must be equipped with the cultural sensitivity and language skills necessary to communicate effectively with families in their mother tongues. Local religious leaders and community elders must be brought on board to help break down misconceptions and offer reassurance.
Polio vaccination drives will not succeed if refusals continue to grow unchecked. Authorities must move beyond the traditional door-to-door campaigns and directly confront the root causes of vaccine hesitancy. Until Pakistan addresses these issues with understanding and strategic engagement, the country’s goal of eradicating polio will remain out of reach, hindered by its own people’s doubts.
